Intermittent dropped calls, busy signal when dialing out, and callers cannot hear randomly

Here is an instance of a dropped call that I am having. The line I'm wondering about is -- DBdel: Error deleting key from database. Also, what is calling the hangup macro? I've read in a few posts that a dial plan can cause this. We recently changed our ring groups from 2000-2099 to 9700-9739. This also changed my dial plan. I'm a noob and this is what my dial plan looks like. It's really specific, because our users wanted the extensions to dial right after dialing.
