IVR kicking in mid-call Trixbox 2.8

Had a stable, nicely working Trixbox 2.8 system with 25 users, Cisco 7940 handsets.

Out of the blue today, mid conversation the customer got the IVR playing and the call hung so neither party could hear each other?

Did a yum update and had 60 updates, all installed. Rebooted / kicked the system and away it went again...

Common problem with known fix? Couldn't see anything on the help forum?
