B400P problem, please help

I'm having a problem with incoming calls.

Sometimes when someone calls, he can't pass IVR menu.

And sometimes when a call is made I can recieve the caller but the caller can't heare me.

This only happens on calls to B400P not on SIP trunk.

Can someone help.

Does this happen on the fixed port or the random port? And what's the frequency does this happen?
