Is there a "best practice" for numbering extensions. I have seen on many Nortel Meridian systems that they often use 2xxx extensions for the "main" extension and 6xxx for backup.

I wrestled with this for a couple of installs, and I've started using 3 digit extensions, starting with 100. Most of the installations I've done have replaced multiple (40-80) POTS phone lines with Asterisk and analog FXO/FXS interface cards and one or more PRI/T1 lines. If I have SIP or IAX extensions, I usually number them as 200+ extensions.
The system examples use extensions starting with 500, but the choice seems to be fairly arbitrary.
The Nortel structure seems to be as good as any I've seen - if you and your customers are comfortable with 2xxx and 6xxx extensions, then I'd say go for it.
You have to adjust the digitmap if you do, not a big deal, but just another thing to do (or forget to do and suffer for).
Start with 200 or 2000.
