DID number lookup from TB mysql

hi everyone can this be done and for how much for anyone interested. we have 100 DID numbers, we want incoming calls to be forwarded to cell phones, example 800 111 1111 DID number should forward to a particular cell phone, but the DID numbers and destination to be stored in the TB mysql database. so in future we can make cell phone number and DID number changes from the TB mysql database and not through TB. Is there any other way if it can not be done? thanks
