Does a SIP phone need to be connected with a cat 5 cable? I am assuming they have an RJ-45 jack, and not and RJ-11, is that correct?

Well, yes and no. You need an 8 position modular jack (aka "RJ45") to connect an IP phone to a switch (and an IP-PBX). However, the PoE standard allows for a minimum of Cat3 cabling. We've tested 100m length of Cat3 between an Aastra IP phone and our switch, and it works without issues.
Despite that, we strongly recommend all our clients move to a minimum of Cat5E cabling for their VoIP system and for future applications.
