I check for updates via module admin and yum updates. But I am really not sure how to upgrade to the latest FreePBX version or Asterisk

First: Trixbox isn't really based on FreePBX anymore. And there is no yum upgrade path between CentOS 4 and 5. You must reinstall.
That said, you can sometimes, in a less than perfectly clean manner, take a FreePBX backup and restore it into a fresh 2.6.x installation. I would back up yourself as many ways as you can (noting that you need to do endpoint manager, HUD, and /tftpboot separately) and install a fresh 2.6 instance. Then restore the data.
Please note - this is NOT clean, nor were the backup / restore modules to date really ever written to be used for upgrading the system. You WILL have occasional issues and you will have to go back and clean things up. The only way to really do this properly is to reinstall and rebuild your system from scratch.
