This is an out of the box 2.8.0.4 install, with 57i phones running the latest firmware. When a VM comes in, the MWI lights, and I get the envelope icon.
However, when I delete the VM, the mwi does not turn off, and the 1 envelope stays.
Any ideas?

I don't know what can happen there, I would agree with obelisk setting the polling in voicemail.conf usually fixes the problem. Wha happens s tha if you use the voice mail script to delete a message, asterisk voice mail is not warned so if polling is not configured the Mwi is not updated, you really have to make sure that it is properly setup, enabled and a poll value of 30s.
The other cause would be that the phone misses the notify message sent by asterisk if send, to check that y can set asterisk with maximum level of verbose and see if the message is sent.
Which firmware version are you using? 3.2.2.
