Polycom IP 501 phones not allowing extentions to be dailed

I have 11 Polycom 501. All of them except 2 are allowing extension dialing. The 2 phones that are not working, I have been working with and have stepped through a number of thought process to fix them. Nothing has worked. I have re-provisioned them. manually setup them up. Rest the phones to default. I am curious if anyone else has run in to this problem and knows the fix.

The phones local dial plan

The phones local dial plan must be set wrong.
